  Use of antibiotics and decongestants

The cause of the development of this disease is infection. To stop its spread, it is necessary to use antibiotics, for adults and children, only a doctor should prescribe them.

Antibiotics are prescribed only in those cases if you have a bacterial form of the disease. If you do not know about the form of the disease and start drinking antibiotics, then as a consequence you will develop complications and side effects.

The bacteria that cause the development of this disease are susceptible to aamoxicillin, so this substance must necessarily be present as part of the medicine for antritis. Effective antibiotics   Amoxyl, Flemoxin Solutab, Ospamox and their analogues are.

Reception of the specified medicines for adults and children does not depend on food intake. The doctor prescribes the dosage, taking into account the age and weight of the patient, as well as the severity of the disease. If a person has a chronic form of the aforementioned ailment, then the dosage of the remedy used for sinusitis increases.

If the sinusitis is caused by fungal or viral infections, then antibiotics cannot be used, here the most effective will be antifungal and antiviral drugs.

Decongestants are prescribed to relieve edema that hit the nasal mucosa. There are many such products now available, in most of them oxylmetazoline is the main substance.

Nasol, Oksilmetazolin, Lekonil and similar drugs which action is based on narrowing are most often used blood vesselsthat reduces blood flow to the nasal passages. This reduces nasal congestion and reduces swelling.

These drugs can be used in adults and children, regardless of what form they have of sinusitis. They are available in the form of drops or spray, and their dosage depends on the age of the patient. Especially effective is the use of decongestants, if the disease is of allergic origin.

  The action of analgesics and mucolytics

A feature of this disease is that it is always accompanied by the presence of pain, usually it is a headache, toothache and pain in the area of the nose.

Analgesics are used to relieve these symptoms. The list of the most effective drugs: Ibuprofen, Aspirin, Naproxen and similar means. Their reception should be carried out clearly in accordance with the doctor's prescription, as they have many restrictions.

If a patient has sinusitis and is less than 20 years old, then aspirin should not be taken. Another feature of taking analgesics is that you can use these drugs for no longer than 5-7 days in a row.

To ensure the removal of mucus and prevent its further education, it is necessary to take mucolytics. The components that are part of the mucolytics, by its action on the mucus cause its liquefaction, as a result of which it is quickly and effectively removed from the sinuses.

Reception of mucolytics is prescribed together with antibiotics and other methods. These are medicines such as Fluuditec, Mukodin, Fluimucil and others. They may be in the form of drops or in the form of a syrup.

Now in pharmacies appeared new medicine Gvayfenezin. The same active ingredient is in such preparations as Mucinex, Robitussin and similar. They are released without a doctor's prescription and have a low cost, Gvayfenezin has almost no side effects, but today the effectiveness of this substance has not yet been proven.

  Use of corticosteroids and herbal remedies

To relieve inflammation of the nasal mucosa, the doctor prescribes corticosteroids. In this case, they can be in the form of a spray or taken during inhalation. The effect of the use of corticosteroids is based on the fact that a group of these drugs acts on the human immune system and increases its functionality.

It is possible to take corticosteroids only strictly according to the doctor's prescription, since these drugs often lead to the development of undesirable side effects. Their appointment is carried out in cases where the treatment by other methods was ineffective, if there is severe inflammation   and edema or in the nose polyps are detected.

Acceptance of corticosteroids is justified in the development of viral, bacterial and allergic sinusitis. They help to quickly deal with inflammation, but when they are taken, one should strictly follow the instructions and recommendations of the doctor.

Phytopreparations are popular as they are of natural origin. Such tools are presented in a wide range, one of the most famous is Sinuforte. However, its price is high, and there are heated debates about efficiency.

In this tool, the main active ingredient is extract of cyclamen tubers. The drug causes a dilution of the inflamed secretion, after which it is removed from the nasal sinuses. The use of this drug is an alternative to puncture and in many cases is effective, but the treatment should be under the supervision of a physician.

To prescribe any medicine should be a doctor based on the development of the disease in each case. If a patient has blocked anastomosis, an increase in mucus secretion will lead to undesirable consequences. Sinuforte and similar drugs can be taken in the case when the normal outflow of mucus through the nose is not disturbed.

Another popular local antibiotic   Isofra is considered an active ingredient in it is framycetin, to which causative agents of sinusitis are sensitive.

It can be used only for children older than 1 year. Before taking it is necessary to dry the nasal vasoconstrictor drops. Isofra can be used for no longer than 10 days, as this may lead to a decrease in immunity and the development of fungal diseases.

The likelihood of recurrence of the disease increases if surgery was performed in the nasal cavity.

There are several types of therapy:

  • nasal drops;
  • antiseptic preparations;
  • bacteriophage solutions;
  • nasal sinus washing solutions;
  • ancillary drugs;
  • antibiotic drugs;
  • surgical intervention;
  • folk therapy.

2 Drug therapy

Like local treatment   has a negative side - there is a narrowing of the vessels in the nasal cavity. This causes the use of drugs for no longer than 5 days. Drops cause quick addiction to the active ingredients, which leads to atrophy of the nasal mucosa with prolonged use. They bury the medicine for sinusitis only with acute manifestations of the disease, when the stuffy nose prevents you from falling asleep. Experts recommend the use of such drops in the nose:

  • "Dr. Theis Nazolin" - drops with an extract of eucalyptus essential oil. They have not only a vasoconstrictor, but also a moisturizing effect, and the eucalyptus oil regenerates and cleans the tissues of the nasal mucosa;
  • "Bioparox" - an antibiotic in the form of an aerosol for local use. It kills the pathogen, which is localized on the mucous membrane and in the sinuses of the nose;

Therefore, tests are required for the presence of an allergic reaction of the patient to the main components of the drug. Pregnant and lactating women are prohibited to use it. If the patient has hypertension, it is not recommended to use Sinuforte;
  • "Rinoprint" - a drug from the group of combined action. Effective for adults, as it contains antihistamines.

Before instilling the nose, it is important to first wash it with saline or herbal solution. It can be bought at the pharmacy or prepared at home. "Aqualore" is considered one of the most effective drugs for washing the nasal cavity. "Dolphin" may be used with preliminary mixing in water. Means after washing it is important to bury in such a sequence: first - vasoconstrictor drops, after them - antibiotics, in the end - drops with antihistamine action.

3 List of antiseptics

Before treating antritis with antiseptics, it is recommended to consult a doctor. The main goal of therapy is the outflow of mucus from the sinuses and their complete cleansing. For this you need to completely clean sinus   antiseptics of a different mode of action:

  • "Dioxidin" - this remedy for sinusitis is represented by a 1% solution in ampoules. Kills a large number of bacteria. Experts do not recommend the use of children and women during pregnancy and lactation;
  • "Miramistin" - the basis of this medicine for sinusitis is chlorine. This solution is not only instillation of the nose, but also its washing. There are no contraindications to this medicine, but doctors do not recommend using it if the patient has allergic reactions;
  • "Furacilin" - the most common solution of 0.02%, used for washing the nasal cavity. The method of application is simple: dissolve 2 tablets of the drug in 1 glass of water. Douching is performed with a pipette. It is important to wash both nasal passages in turn. It is necessary to avoid accidental swallowing of the solution during the procedure. But if the patient swallows a small amount of the solution, there will be nothing terrible. For complete recovery, it is enough to hold about 10 washing sessions;
  • "Protargol" - a drug based on a solution of silver. Bury up to 3 drops in each nasal passage. Pharmacists prepare the medication by prescription. The duration of therapy is up to 10 days. It is used for the treatment of sinusitis in adults and children.

4 Bacteriophage solutions

The best cure for sinusitis is a bacteriophage. It is used alone in the form of antibiotic agents or for washing the nose. It has a neutralizing effect against pathological bacteria.

Bacteriophages are effective only in case of antritis of the bacterial form. Because of this, before their appointment, a competent specialist assigns an analysis for bacterial culture to determine the causative agent and the degree of its sensitivity to medicines of various forms and groups.

Drugs of this group are also used to treat children in whom the disease is caused by a staphylococcal infection. Sinusitis in adults is also often treated with bacteriophages in combination with antibiotics. Bacteriophage solution should be stored in a freezer and warm to room temperature before use.

Rinsing the nose at home is carried out using a solution salt, it can be bought at the pharmacy. For the treatment of sinusitis use special solutions - "Aquamaris", "Aqualor" and others. The use of such drugs is very convenient, since they do not need to be prepared each time before use.

5 Auxiliary drugs

Acute sinusitis is characteristic not only of the stuffy nose, but also elevated body temperature. Experts recommend antipyretic and anti-inflammatory drugs. The most common are Nurofen and Ibuprofen. To reduce the swelling of the nasal mucosa, you need to take drugs that contain loratadine.

Before the appointment of treatment of sinusitis should be conducted research. Therefore, before therapy, it is important to determine the genus of microorganisms and their sensitivity to antibiotics. For this purpose, conducts the sowing of mucus from the nasal sinus.

If the patient does not have allergic reactions to antibiotics, doctors prescribe a broad spectrum antibiotic drugs. These drugs are prescribed in the presence of discharge from the nose of a yellow or green color, elevated temperature   body.

6 Surgical intervention

There is a possibility of a puncture of the sinuses and suction of pathogenic contents from them. But such a procedure has serious consequences in 75% of cases. Therefore, developed two methods of treatment without drainage of the sinuses:

  • yamik-catheter - is carried out using a special catheter made of soft materials. First, the doctor performs anesthesia. In one nostril there is a catheter with a balloon at the end. The second cartridge is located in the middle of the tube. After insertion of the catheter, the second balloon is sticking out of the nose. When inflating the nasal and sinus cavities are sealed. The standard catheter has 2 exits for the syringe. By means of the first, various manipulations with pressure in the sinuses are performed. The second exit is used to enter drugs and antiseptics. This type of surgery is possible for children and pregnant women, since it can be performed without the use of strong and unsafe medicines;
  • balloon sinusoplasty is the introduction of a balloon-type catheter into the sinus of the nose and the inflation of a small plastic bladder at the entrance to the sinus. Surgery helps to expand the closed axillary passage. Symptoms of sinusitis ( headache, pressure and mucus from the nose) is eliminated after a similar procedure.

  • mechanical cleansing of clots of purulent secretions;
  • moisturizing the nasal mucosa;
  • improvement of the cilia of the mucociliary apparatus;
  • elimination of inflammatory foci.

The following drugs can be used to perform them:

  • antiseptics: Miramistin, Furacillin, Chlorhexidine, Dioxidine solutions;
  • dolphin powder (for solution preparation);
  • based nasal sprays sea salt   for irrigation of the nose: Marimer, Aqua Maris, Salin, Humer, Aqualore and others;
  • hypo-, iso- and hypertonic solutions with the addition of propolis tincture, calendula, etc.

The effectiveness of washing the nose for the treatment of sinusitis and other sinusitis is recommended by the leading otolaryngologists of the world and is actively used to treat adults and children of any age.

Corticosteroids

  Nasonex

Preparations from the group of corticosteroids can significantly alleviate the condition of patients with viral, bacterial or allergic sinusitis. They have a pronounced anti-inflammatory effect and contribute to the restoration of the immune forces of the body. The main indications for corticosteroids are:

  • pronounced inflammatory focus and swelling of the nasal mucosa;
  • lack of effect from the use of other drugs;
  • the presence of polyps in the nasal cavity.

The most popular corticosteroid drugs for the treatment of sinusitis are:

Dosing of corticosteroids and the duration of their use can only be determined by a qualified ENT doctor who can take into account the severity of sinusitis and the particular health of the patient.

Analgesics

  Ibuprofen

The use of analgesics for antritis helps to improve the general condition of the patient and eliminate headache and temperature. For this, non-narcotic analgesics can be used, which also have a pronounced anti-inflammatory effect on the nasal mucosa. Most often for these purposes are assigned:

  • Ibuprofen;
  • Nurofen;
  • Acetomato;
  • Aspirin.

The duration of taking these medicines should not last more than 7 days, and before using them you should consult with your doctor about the safety of their use. Analgesics are prescribed with caution in patients with gastrointestinal ulcers, allergic reactions   and asthma.

Mucolytics

  Fluimucil - mucolytic

The prolonged course of the inflammatory process in the maxillary sinuses leads to an increase in the viscosity of the nasal secretion. To dilute it, it is extremely important to apply mucolytic agents that contribute to the dilution of thick pus and mucous secretions:

  • Fluimucil (tablets, solution) - the active ingredient of the drug is acetylcysteine, which contributes to the rupture of intra- and intermolecular disulfide bonds of sputum mucopolysaccharides and leads to a decrease in sputum viscosity, and the drug can have an antioxidant and anti-inflammatory effect, can be used to treat children of any age group (for children under 6 years old are recommended to use only the tablet form, and the solution can be used only in the hospital);
  • Rinofluimucil (spray) - the drug is a combination, its active ingredients are acetylcysteine and tuaminoheptan sulfate, which have mucolytic, vasoconstrictor and antioxidant effects, the drug can be used for children of any age, but children under 3 years of age are prescribed with caution.
  • Fluditec (Mucosol, Fluifort, Mukodin) - the active component of the drug is carbocysteine, which helps to dilute sputum and purulent contents of the sinuses, restores the secretion of immunoglobulin IgA, the drug is available in the form of syrup and can be used to treat children over 2 years old.

Antihistamines

  Claritin Antihistamine

When antritis antihistamines are used to reduce the swelling of the nasal mucosa. To do this, the patient is prescribed drugs of group II or III, which have a lesser inhibitory effect on the central nervous system:

  • Loratadin (Lorano, Claritin, Lorfast, and others);
  • Sequifenadine;
  • Phenystyle;
  • Fencarol;
  • Cetrin (Aleron, Cetirizine, Zodak, Zyrtec, etc.);
  • Desloratadine (Eden, Erius, Freebris);
  • Fexofenadine (Telfast, Tigofast, Allergo, Altiva);
  • Levocetirizine (Aleron, L-Zet, Xizal).

Herbal preparations

  Cinnabsin

For the treatment of acute and chronic sinusitis, combination preparations based on natural ingredients that can influence the symptoms of the disease are successfully used. The most famous among them are:

  • Cinnabsin (tablets) - homeopathic medicine, contributing to the elimination of headaches, swelling of the mucous membrane, difficulty in nasal breathing, inflammatory foci and neuralgia trigeminal nerveThe components that make up the drug reduce the viscosity of secretions and have an effect on the body; it can be used to treat children;
  • Sinuforte (nasal aerosol) - a drug based on lyophilized juice and an extract of European cyclamen tubers, can act on the trigeminal nerve receptors and, after 30-60 minutes, cause a reflex increase in secretion in the mucous membrane, hypersecretion leads to liquefaction and an increase in the volume of inflammatory exudate, which facilitate the evacuation of secretions from maxillary cavity, also the drug has anti-inflammatory and vasoconstrictor effects, can be used to treat children over 5 years old;
  • Sinupret (pills, drops for oral administration) - primrose, elderberry, gentian, sorrel and verbena powders are included in this herbal preparation, due to their properties, it is able to have mucolytic, secretive motor, anti-inflammatory, anti-edematous and immunomodulatory effects, can be used as Pills for the treatment of children over 2 years, in the form of drops - over 6 years.

Which doctor to contact

When sinusitis is necessary to seek help from a therapist or ENT doctor. If necessary, a consultation with an allergist and a physiotherapist will be scheduled. In severe cases of antritis, treatment with maxillofacial surgeon may be necessary.

Dr. Komarovsky tells about the treatment of sinusitis in children.

Sinusitis - quite common, and at the same time very dangerous complication common cold. In the treatment of this disease, the central role is played by drugs of general effect - various tablets for sinusitis - antibacterial and anti-inflammatory drugs, as well as drugs local action   - solutions for washing, drops and sprays. In this article we will describe in detail what drugs are used to treat sinusitis, and what is their effect.

Sinusitis is a fairly common and very dangerous complication of the common cold. In the treatment of this disease, the central role is played by drugs of general effect - various tablets for antritis - antibacterial and anti-inflammatory drugs, as well as local drugs - solutions for washing, drops and sprays. In this article we will describe in detail what drugs are used for, and what is their effect.

It should be borne in mind that antritis is a serious disease that can lead to serious complications, and in some cases to death. Because of this, self-treatment for sinus is excluded - all their actions should be discussed with the doctor beforehand.

Sinusitis - what is it?

Sinusitis is called inflammation maxillary sinuses   the nose. This is one of the variants of sinusitis (inflammation of the paranasal sinuses).

Viruses and bacteria get into the sinuses quite often, along with inhaled air. It is not dangerous if the outflow of mucus from the sinuses is not disturbed, and the mucus itself is sufficiently liquid. However, with inflammation of the mucous membrane (with ARVI, rhinitis), the walls of the sinuses increase in volume and may overlap the mouth of the sinus. As a result, bacteria begin to actively proliferate in the sinus, mucus and pus accumulate. The waste products of bacteria enter the bloodstream, causing intoxication. This is accompanied by a violent reaction from the immune system, in particular, a significant increase in body temperature.

The main causative agents of sinusitis (as well as other types of sinusitis) are:

  • in the acute form - streptococcus (about 50% of cases), hemophilus bacillus (12%); pathogens such as moraxella, Staphylococcus aureus, various anaerobes are much less common;
  • in chronic sinusitis, anaerobic bacteria (Peptococcus, Bacteroides, Veillonella, Prevotella, Fusobacterium, Corynebacterium) are found more often, rarely golden staphylococcus, pneumococcus, hemophilic bacillus, and also microscopic fungi.

Clinical picture

Patients suffering from sinusitis complain of the following symptoms:

  • feeling of pressure in the sinuses, "fullness", "bursting" in the face;
  • headache, which becomes sharper when tilting or sharply turning the head;
  • pain when pressed on the projection of the maxillary sinuses (on the sides of the wings of the nose);
  • high body temperature (above 380 ° C);
  •   (may be absent if the mouth of the sinus is completely blocked);
  • pus can also drain down the pharynx;
  • nasal congestion (not always).

Sinusitis can be unilateral (inflamed one sinus), or bilateral (affected both sinuses).

Treatment - the general scheme

Treatment of sinusitis includes several areas:

  • the destruction of the infection inside the inflamed sinus;
  • improvement of the outflow of mucus from the sinus, the elimination of pus;
  • removal of inflammation in the affected mucous area;
  • fight against intoxication accompanying acute inflammatory process;
  • relief of patient health.

Thus, it is not enough to use only one drug to cure sinusitis completely - the disease must be treated in a complex way.

Let's talk about what drugs should be used in the treatment of sinusitis, what is their role, and whether it is possible to do without some of them.

Drugs in pills

Acceptance of drugs of general effect (tablets, solutions and powders for injection, etc.) are the generally accepted approach in the treatment of sinusitis today - practice shows that it is impossible to limit yourself to local means. Contrary to popular myth, tablets for sinus are no less effective than injections.

At the same time, injections are resorted to in the event that the patient’s condition is severe, and it is necessary to ensure that the medication is rapidly released into the blood. Intramuscular or intravenous   should be practiced only in a hospital, but not on an outpatient basis. If the treatment is started in a timely manner, and the patient's condition is satisfactory, preference is given to pills - their reception causes much less discomfortand in some cases this factor is not important (for example, if we are talking about a sick child).

Antibiotics

Treatment of sinusitis is not complete without antibiotics, as they affect the root cause of the disease - a bacterial infection.

And if the bacterial rhinitis is still really cured without them, resorting to frequent washing and active use of antiseptics, then for sinus remedies such procedures are practically useless - a very small amount of medication injected through the nose gets into the sinus cavity, what can we say about attempts to wash out the pus. The only solution is to act on the infection through the blood.

A list of antibiotics used in the treatment of sinusitis is presented in Table 1. It is worth noting that antibiotics of the first choice for the treatment of acute sinusitis are drugs based on amoxicillin and ampicillin; the rest are resorted to in case of individual intolerance to penicillins, or if the pathogen is resistant to these drugs.

Antibiotic Trade names Dose of administration A course of treatment Spectrum of action
Penicillins

Table 1 Antibiotics used in the treatment of sinusitis, including sinusitis

To make antibiotic treatment as effective as possible, it is recommended that you pass bacteriological seeding of nasal secretions before prescribing them. This study provides information on the types of bacteria that caused the infection, and also shows their sensitivity to different types antibiotics. At the same time, it is impossible not to note the fact that the standard growth time of a bacterial culture is 4-5 days; you shouldn’t let purulent disease take its course for such a substantial period of time, so the doctor usually prescribes an antibiotic immediately, based on the symptoms of the disease, as well as the patient’s history of antibiotic therapy. Bacteriological research is resorted to if the prescribed tablets for sinusitis were ineffective, or if the patient is faced with multiple episodes of sinusitis (for example, has sinusitis after each runny nose, more often 2 times a year). In addition, bacteriological sowing of mucus should be a mandatory step in the treatment of chronic sinus inflammation.

Anti-inflammatory

Another group of drugs, without which the treatment of sinusitis is not costing - anti-inflammatory tablets. It is worth noting that this is only an auxiliary treatment, since anti-inflammatory drugs relieve the symptoms of inflammation, but do not affect its cause - the infection.

Anti-inflammatory drugs mask the presence of bacteria in the body, but they do not help get rid of them.

However, anti-inflammatory - good helpers in the fight against the effects of intoxication. They help to quickly remove the heat, reduce headaches. In severe condition, they are simply irreplaceable. Thus, they should be taken as needed - if the body temperature exceeds 38 degrees, severe headache worries, it is impossible to fall asleep, etc.

What anti-inflammatory tablets can be used for sinusitis? If the disease is medium severe course, you can take paracetamol. This medicine is presented in pharmacies under various names - Panadol, Tylenol, Ifimol, Valadol, Efferalgan, etc. The drug is taken after meals with plenty of water. There are many drugs based on paracetamol for children.

If sinusitis has a severe course, paracetamol will not always be able to cope with the fever. Need more strong drug   - ibuprofen. Children under 6 years old are contraindicated.

It should be noted that with the proper selection of antibiotics, body temperature returns to normal within 1-2 days of administration, and the need for anti-inflammatory disappears.

Topical preparations

Topical preparations - drops, sprays and solutions for washing - improve the flow of mucus in the nasal cavity, remove the pus from the nasopharynx and thus, accelerate recovery from sinus.

Nasal drugs used in the treatment of sinusitis:

In addition to drops, sinusitis is recommended for washing the nasal passages with antiseptic solutions. For example, Dioxidine solution (1%) is used for nasal showers for sinus, using a syringe without a needle or a small pear. In this case, the solution flowing into the pharynx should be spat out - Dioxidine entering the stomach is undesirable. Similarly, antiseptics Miramistin and Furacillin are used.

Inhale, drink hot drinks and warm the nose area while sharp sinus   strictly contraindicated.

When is surgery required?

With timely treatment to the doctor and careful adherence to prescribed treatment, sinusitis is quite well drug treatment. However, it cannot be guaranteed that it is possible to cure antritis without surgery in 100% of cases. First of all, this applies to patients with impaired drainage of the maxillary sinuses. For example, if a person has a curved nasal septum, the outflow of mucus from the sinuses will be incomplete, pus will accumulate, despite taking a large amount of medication.

The same thing happens if the mouth of the sinus is blocked by a polyp or a cyst. In such cases, the doctor recommends puncture. maxillary sinus. This is a small operation during which the bone wall of the inflamed sinus is punctured with the help of a special needle. Through the hole formed pus. Next, the doctor rinses the sinus cavity with an antiseptic and injects the antibiotic directly into it. If you need multiple washes, a catheter is inserted into the sinus (a flexible tube through which an additional antibiotic will be injected).

Also, the operation has to be resorted to in neglected cases of sinusitis, if on the radiograph, the doctor sees that the sinus (or sinuses) are filled with a large amount of pus. If the patient is in serious condition, it is better not to abandon the operation - as soon as the pus comes out, the body temperature will decrease, and the acute headache will disappear. Subsequent antibiotic treatment will help to quickly overcome the infection and achieve complete recovery.
